Commissioning body
Try to discuss your concerns with the consultant. However, we understand that this is not always possible. More serious complaints should be reported directly to the complaints team at C-CAF.
You can make a complaint by accessing the C-CAF online complaints form.
You should make your complaint within 12 months of the incident or when the matter came to your attention. When writing a complaint, you can include:
The most important matters first
An explanation of why you are not satisfied
Clear and brief language
Numbered or bulleted points
Questions you would like answered, in order of importance
The outcome of your investigations and gather evidence.
Your contact details, including your address, phone number, and email address
You can expect your complaint to be acknowledged
